Eastern Shore Rural Health System, Inc. is a Community Health Center network seeking Nurse Practitioners. We are a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). We employ National Health Service Corps participants.

We provide medical, oral, and behavioral health care and support services to everyone in our community regardless of ability to pay. As primary health care providers, we serve our patients and work with others to manage patients’ health needs in a safe, professional way that also respects the culture of every individual.

Serving Virginia’s Eastern Shore with four medical centers and five dental locations, we are the medical primary care provider of choice. In addition to primary medical and dental care, we offer programs including health education, help obtaining prescriptions for those who can’t afford them on their own, digital x-ray, labwork, travel immunizations and more.  Our electronic health records mean you can be seen at any center and your complete medical history will be available.

1. Will provide primary care services of an acute, chronic and preventive nature to patients of all age groups as guided by organizational protocols and license to practice.
2. Will function as an interdependent member of the health care team with both dependent and independent functions. Supports and assists the center clinical staff. In the absence of the Clinical Director with no other physicians available will act as Center Supervisor within the confines of the Virginia Nurse Practitioner Act.
3. Adheres to quality assessment guidelines, OSHA or other regulations as federally mandated or as set forth by principles of practice of ESRHS.
4. Maintains certifications and keeps all licenses current, and provides evidence of ongoing CME (if CME is requested).
5. Maintains certification as Basic Life Support Provider. Additional certifications may be required.
6. As part of the center team, consisting of other providers, nurses, health education specialists, case managers, interpreters, transportation and outreach workers and others, assists in implementation of the clinical services plan with measurable goals and objectives as outlined in the Health Care Plan of the Federal 330 Grant.
7. Accepts clinical responsibilities/duties assigned or requested by the Chief Medical Officer or Chief Executive Officer.
8. Assists in the responsibility of call for the practice.
9. Abides by policies, procedures and protocols as needed to achieve the center’s goals and objectives.
10. Represents ESRHS in the community through involvement in community activities which support the mission and philosophy of ESRHS.
11. Maintains organized, complete electronic medical records with regard to content appropriateness and confidentiality.
12. Participates in center staff meetings, sharing responsibility for effectiveness of these meetings when requested with center manager and clinical director. Supports and assists the clinical staff as needed, makes recommendations for service improvement, patient flow and changes to center.
13. Shares in responsibility of teaching of students, residents, and other health professionals as the community health center becomes involved in the training of primary care providers.
14. Attends quarterly ESRHS provider meetings unless prevented by unusual need of center functions or hospital call duties. If unable to attend, must notify CMO for approval. Every other quarterly provider meeting may be viewed at the medical provider's center.
15. Must demonstrate excellent internal and external customer service skills.
16. As part of the Patient Centered Medical Home Model of care, the employee will advocate for patient’s best interest and make recommendations for patient special needs such as interpreter services; identified barriers to care such as transportation, medication assistance, outreach, and health education; influence patient’s engagement in their care; promoting a continuum of safe, quality, and cost effective patient care.
17. Employee will abide by the Code of Conduct as documented in the Corporate Compliance Manual.
18. Employee must demonstrate an understanding of the intent, and follow standards, guidelines and protocols related to achieving and maintaining certifications by outside agencies.
19. Must possess the ability to follow supervisor’s instructions and maintain regular attendance.
20. Management may modify, add or remove essential job functions as necessary, or as changing organizational needs require.
21. Maintains respect for confidential information gained through employment at ESRHS, unless legally authorized or required to divulge such information.
22. Must demonstrate personal and professional commitment Eastern Shore Rural Health System and its Mission Statement.

Requirements

Individual must meet the following requirements: 

  • Registered Nurse Licensure
  • Graduate of an approved Master’s level or Doctorate's Level course of Nursing Practitioner education
  • Certification by a National Nursing Organization
  • Certification and Licensure by Board of Nursing
  • Two years work experience preferred.

Individual must possess the ability and skills to plan, organize and implement clinical programs and activities as directed by the Clinical Director.
Ability to work well cooperatively with diverse groups of health professionals and center staff; ability to work independently and make
appropriate decisions which reflect professional strength, confidence and integrity.
Nurse Practitioners are privileged with age restrictions according to their appropriate scope of practice. 

Physical Requirements:

  • While performing the duties of this job, the employee is regularly required to talk and hear.
  • This position is very active and requires standing, walking, bending, kneeling and stooping all day.This employee must frequently lift and/or move items over 30 pounds.
  • Must be able to lift, carry and handle equipment, supplies and other work site materials according to position requirements. 

The Eastern Shore of Virginia is a peninsula extending 70 miles from the Maryland state line south to the Chesapeake Bay Bridge-Tunnel. To the east are the “seaside” marshes, shallow bays and barrier islands bordering the Atlantic Ocean. On the west, or “bayside” are the creeks, large and small, flowing into the Chesapeake Bay. The peninsula is divided into two counties: Accomack County (population 33,413) to the north and Northampton County (population 12,282) to the south. Picturesque towns dot this land between the two waters in close proximity to the Hampton Roads, Virginia region.

Miles of unspoiled ocean beaches on the barrier islands that line the coast and the region’s numerous outdoor activities continue to attract visitors and residents alike. Recreational opportunities abound, like hiking, biking, camping, boating, swimming, crabbing, fishing, or simply observing wildlife in its natural setting while enjoying the 9,000-acre Chincoteague National Wildlife Refuge.
